Home Health

WinnMed Home Health provides medical services to you and your family in your own home, offering an alternative to costly long-term stays in a hospital or nursing home.

Home health care is available if you are managing chronic conditions, such as congestive heart failure or diabetes, or for acute medical conditions, such as after a stroke or a surgical procedure. Based on your individual needs, your home health care team can consist of nurses, home health aides, physical or occupational therapists, as well as a social worker.  Nurses monitor your medical condition, assist with managing medications and provide education regarding your medical diagnosis. Home health aides provide assistance with personal care, such as bathing or hair-washing, and may also provide a limited amount of housekeeping assistance. Therapists assist patients with strengthening and enhancing the safety and accessibility of your home environment.

As a home health patient, your needs are assessed and a personal care plan is designed under the direction of your own physician.

Nurses are on call 24 hours a day, seven days a week.

Referrals may be made by physicians, nurses, other health care providers, family members, friends, clergy or patients themselves.

Cost

WinnMed Home Health is a Medicare certified agency. Therefore, Medicare, Medicaid or private insurance can be billed for these services. No one will be denied home health care because of their inability to pay. Charity care may be available.
