Vaccine Administration Record Form and Checklist
- Winneshiek Medical Center has Pfizer vaccine available for age 5+
- Winneshiek Medical Center has Moderna vaccine available for age 18+
- WMC has booster vaccine available for Pfizer and Moderna.
- Anyone who wants to receive the vaccine at Winneshiek Medical Center in Decorah can call us at 563-382-2911 for a vaccine appointment.
- You do not need to be a patient of Winneshiek Medical Center or Mayo Clinic Health System to receive the vaccine through us.
- If you have received your first dose vaccine through WMC, we will call you to schedule the second dose.
For patients with a vaccine appointment: Please print, fill out and bring the Vaccine Administration Record Form and Checklist to your scheduled vaccine appointment.
